Occupational Therapist

Reporting to the Manager Support at Home, the Occupational Therapist works as part of a multidisciplinary team comprising management, Coordinators, Support Workers and other healthcare professionals to deliver high-quality, person-centred allied health services.
The Occupational Therapist provides evidence-based assessment, intervention and clinical recommendations to support older people receiving services through the Support at Home program and other aged care initiatives. Delivering services across both clinic-based and community settings, the role focuses on promoting independence, enhancing safety and wellbeing, supporting reablement and restorative care, and enabling clients to achieve their individual goals while remaining living safely and independently in their own homes.
In addition to therapeutic responsibilities, the Occupational Therapist is expected to act as a subject matter expert by providing educational opportunities, advice and leadership, as they pertain to the provision of care to clients of Community Based Support.
Key responsibilities:
- Provide high-quality Occupational Therapy assessment and intervention for clients in a professional and confidential manner,
aligning with best practice standards. This includes, but is not limited to, clinical assessments and home visits, equipment prescription, rehabilitation and occupational therapy.
- Ensure integrated service delivery and follow up, coordinating and communicating as required with other relevant internal and external providers.
- Incorporate a holistic approach to health care consistent with client, community and organisational priorities.
- Work closely with management to support regular tracking of progress in monitoring client progress and care, ensuring feedback is provided to relevant stakeholders, including supporting the implementation of change and identifying care gaps.
- Provision of care, support, education and advice to clients/carers.
- Conducting physical and cognitive assessments of clients and developing or following a treatment plan.
- Assessment of various environments, including in home and workplaces, with the view to determine solutions, adjustments and equipment to support client needs.
- Provide advice and support to CBS staff in areas relevant to Occupational Therapy service provision as required.
- Participate in quality and service improvement activities to continually improve client care including providing input into the development of clinical practices, procedures and protocols related to the provision of Occupational Therapy services in a home care setting.

What you bring:
- Bachelor of Applied Science (Occupational Therapy) or equivalent
- Current registration with AHPRA
- Current Working with Vulnerable People Registration (or ability to obtain)
- A National Police Check that is no older than 3 months (or ability to obtain)
- Driver's licence
- First Aid and CPR certificate
