Australian Unity is seeking a Wellbeing and Transitions Manager based in Mornington to support seamless transitions for current and prospective residents to Peninsula Grange. You will report to the National Wellbeing and Transitions Manager and work within the Better Together model of ageing well.
You will promote a continuum of care, maintain occupancy targets, engage with health professionals and community groups, and build solid relationships across teams and service providers.
