Join Western District Health Service as a Health Care Worker where you'll play a vital role in delivering person-centred care within a supportive and dementia-friendly aged care environment.

What You'll Be Doing

As a valued member of our care team, you'll contribute to the delivery of safe, respectful, and person-centred care across:

Person-Centred Care

Provide individualised care that promotes dignity, choice, independence, and wellbeing, while supporting residents in accordance with their care plans and personal preferences.

Daily Living Support

Assist residents with personal care needs including hygiene, grooming, dressing, toileting, mobility, nutrition, and hydration, ensuring comfort and dignity at all times.

Monitoring & Escalation

Observe and report changes in residents' physical, emotional, or behavioural wellbeing, ensuring concerns are promptly communicated to nursing staff for appropriate action.

Consumer Engagement

Build positive and respectful relationships with residents, families, and carers, supporting emotional, social, cultural, and lifestyle needs through compassionate care and communication.

Teamwork & Collaboration

Work collaboratively with Registered Nurses, Enrolled Nurses, and the broader multidisciplinary team to support the delivery of high-quality care and positive resident outcomes.

Safety, Quality & Documentation

Maintain accurate documentation, follow organisational policies and procedures, and contribute to a safe environment through adherence to infection prevention, manual handling, and workplace safety practices.





Continuous Improvement

Participate in education, training, and quality improvement initiatives, supporting excellence in care delivery and ongoing professional development.

What You Bring

- Certificate III in Individual Support (Ageing), Health Services Assistance, or equivalent qualification
- Experience in aged care, disability, or health care settings (desirable)
- A genuine commitment to person-centred care and supporting older people
- Strong communication and interpersonal skills
- Ability to work effectively within a multidisciplinary team
- Compassionate, respectful, and professional approach to care delivery
- Ability to recognise and report changes in resident condition or wellbeing
- Understanding of infection prevention, workplace safety, and manual handling principles
- Commitment to ongoing learning, professional development, and quality improvement
- Knowledge of privacy, confidentiality, and professional conduct requirements
