The Royal Flying Doctor Service South Eastern Section(RFDSSE) is part of one of Australia's most respected and trustedorganisations, providing 24-hour emergency aeromedical retrievals and essentialhealthcare to people living and working in remote, rural, and regionalcommunities. As the contracted provider for the Western NSW Primary HealthNetwork (PHN), RFDSSE delivers the Keeping Healthy Puura Manti Program (KHPMP)- a time limited initiative designed to improve chronic disease outcomes acrossthe region.

The program strengthens access to culturally appropriate,integrated care by working alongside primary care providers, allied health, andcommunity services to enhance health literacy, self-management, and overallwellbeing.

About the Opportunity:

As an Aboriginal Health Practitioner, you will provide essential culturally safe and clinicallysupportive care for KHPMP clients, particularly Aboriginal and Torres StraitIslander people living with chronic disease. Youwill support clients to access services, understand their care, and stayengaged in treatment, while advocating for their needs. This role is offered on a Maximum Term, Full-Time Contract, based in Broken Hill until 30 June 2027.

Key Responsibilities:

- Deliver culturally protected, respectful, andresponsive care to Aboriginal clients and communities
- Support access to healthcare services,referrals and chronic disease programs
- Provide health education and promoteself-management and health literacy
- Advocate for clients and support informeddecision-making about care and treatment
- Work collaboratively with multidisciplinaryteams, clinics, and stakeholders
- Support care coordination, including attendingappointments where appropriate




- Support and participate in the planning,development and provision of community engagement and health promotionactivities
- Maintain accurate clinical documentation anduphold confidentiality and professional standards
- Contribute to continuous improvement andculturally appropriate service delivery
- Certificate IV Aboriginal Primary Health Care (Practice) with current registration as an Aboriginal Health Practitioner with the Australian Health Practitioner Regulation Agency (AHPRA)
- Strong cultural knowledge and connection tocommunity
- Experience working in health or communityservice delivery
- Excellent communication and interpersonalskills
- Ability and willingness to travel
- Ability to work effectively within amultidisciplinary team
- Proficient in Microsoft Office applications,including Outlook

Mandatory Requirements:

- NSW Working With Children Check (or ability toobtain)
- Category A vaccinations
- National Police Check
