About the Role
We are seeking a motivated individual to join our team as a CHSP Coordinator on a part-time (3 days per week) basis. This role is responsible for coordinating the day-to-day delivery of Commonwealth Home Support Programme (CHSP) Centre-Based Respite and Group Social Support services. The successful applicant will work closely with participants, families, staff and volunteers to coordinate service delivery, support participant wellbeing, and contribute to the ongoing quality and continuous improvement of the program. The CHSP services will be conducted at our Chatswood, Hornsby and Pennant Hills centres.
Key Responsibilities
Coordinate the day-to-day delivery of Centre-Based Respite and Group Social Support services.
Plan, coordinate and deliver day centre programs and community outings.
Coordinate participant intake, service commencement and ongoing service reviews.
Coordinate staff, volunteers,
transport and other resources to support service delivery.
Maintain participant records and program documentation in accordance with CHSP requirements.
Support compliance with CHSP requirements, the strengthened Aged Care Quality Standards and organisational policies.
Contribute to quality improvement initiatives and continuous service improvement.
About You
To be successful in this role, you will have:
Relevant qualifications in Community Services, Aged Care, Social Work or a related discipline.
Demonstrated experience in aged care or community services, including a sound knowledge of the Commonwealth Home Support Programme (CHSP) and its service delivery requirements.
Demonstrated initiative, professionalism and sound problem-solving skills.
Proven ability to work collaboratively and build positive relationships with participants, carers and colleagues.
Robust computer skills, including proficiency in Microsoft Office applications.
Current National Criminal History Check and a current NSW Driver Licence.
Bilingual Mandarin and Cantonese skills are highly regarded.
Requirements
Undergo and successfully complete a 6-month probationary period.
