The Role
The Aunty Grace Care Manager is a critical member of our clients’ Support Team, working closely with each client to assess their overall health, function and independence, and to establish their goals, needs and preferences. The Care Manager conducts all in-home assessments for their clients, and partners with them to build a planned schedule of services to meet the needs and goals of our clients. This role requires an approach that is truly person‑centred and relationship driven.
While the Care Manager has broad responsibility for managing the care of our clients, you will be well supported by colleagues and other health professionals to provide best practice care. In addition to this, you will have the opportunity to work with industry‑leading systems and technology to reduce your admin time and focus on providing great care.
The Care Manager works in a close‑knit team of colleagues and Suppliers to monitor clients’ health and wellbeing, and ensure that services being delivered continue to meet the changing needs and goals of every client. Strong time management, prioritisation and organisational skills are required. The Care Manager must be able to identify where referral for further assessment is required,
and work with a team of Registered Nurses and/or health professionals to manage clinical care and clients with complex needs.
To be a Care Manager with Aunty Grace, you must have:
At least 2 years’ experience in care or case management in the Health, Aged Care or Community Services Sectors; OR Formal qualification (Certificate III or Diploma) in Care Management or Case Management; OR AHPRA registration as an RN or Allied Health Professional will be considered
Experience working within or demonstrated understanding of the Strengthened Aged Care Quality Standards framework
A National Criminal History Check / Police Check showing no disclosable Court Outcomes or Convictions, and no greater than 3 years since the date of issue
A current, valid Australian Driver’s License
Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing or Allied Health discipline with a current AHPRA registration, or equivalent experience
Ambulance/Paramedic qualification or experience
Experience working in case management or care management in either Home Care Packages/Support at Home Program or similar
