Central Adelaide Local Health Network (CALHN) invites an experienced Occupational Therapist to join the Integrated Care Geriatrics team in Sefton Park. You will help older people maintain independence, build resilience and achieve positive health outcomes in their homes or community settings.

You will perform comprehensive assessments, coordinate care and support hospital avoidance through restorative, person-centred interventions.
