What you'll be doing

The Sustaining NSW Families (SNF) Plus pilot program is a nurse-led, structured sustained health home visiting program for children and families from pregnancy up to 2 years of age.

Provide specialist social work and counselling interventions as part of the SNF Plus multidisciplinary team.

Focus on early intervention, that aims to promote child health and development.

Help families work together to build stronger bonds and support parent health and wellbeing.
