All Care Nursing (ACN) is currently partnering with an aged care provider to recruit an experienced Clinical Care Coordinator for their well-established 70-bed home located in the Foster, South Gippsland VIC.

The provider is a community based, not for profit, residential aged care service. Providing ageing in place residential care services as well as short-term respite care for the local community. The 70-bed home is fully accredited to Government standards.

Key responsibilities
- Ensuring high-quality professional and clinical care for all residents.
- Supporting the Care Manager in oversight of care operations, HR activities, staff supervision and development.
- Coordinating and monitoring care processes, assessments, AN-ACC assessments, risk management, and clinical governance
- Providing mentoring, coaching, and on-the-floor leadership to nursing and care teams.
- Delivering and supporting orientation, staff education, clinical training, and continuous improvement.
- Acting as Infection Prevention and Control (IPC) Lead or supporting that function as required.
- Liaising with doctors, allied health providers, families, contractors, and external services.
- Acting on behalf of the Care Manager in their absence
- Current AHPRA registration as Registered Nurse.
- 2-4 years of relevant work experience for this role
- Experience in aged care clinical leadership roles.
- Working knowledge of Strengthened Aged Care Quality, legislation, IPC, and quality frameworks.
- Clinical assessment skills and understanding of Ageing in Place.
- Strong communication, interpersonal and documentation skills.
- Demonstrated initiative, autonomy, and ability to work in a team.
- Strong organisational, time management and prioritisation skills.
- Commitment to diversity, inclusion, and resident‑centred care.
- Full working rights.
- Preferred availability on weekends
- Previous experience as an Infection Prevention & Control Lead
- Previous experience with AN-ACC Assessments and funding
