Omnia Community Health in Sunbury is seeking a dedicated community support role to reduce social isolation and help clients participate in local activities. You will work with clients in centre-based and community programs, and provide one-on-one support for residents living in the community.

Successful candidates will build rapport, communicate effectively, plan and problem-solve, and be willing to drive a 12-seat bus to transport participants on outings.
