The Royal Children's Hospital Melbourne – Parkville VIC is seeking a part time, fixed-term Transition of Care Coordinator (0.4 EFT, 30.4 hours per fortnight) in the Department of Adolescent Medicine. You will provide consultation, guidance, education and transition planning for RCH patients while participating in combined RCH and adult service transition clinics, research and events.

You will coordinate care across teams, maintain accurate notes in the EMR, track patient journeys, and deliver
