As a Credentialled Diabetes Educator, you'll play a vital role in supporting individuals living with diabetes through education, clinical management and ongoing care. Working as part of a multidisciplinary team, you'll provide evidence-based diabetes education, promote self-management, and support improved health outcomes for patients across a variety of healthcare settings.
Your responsibilities may include:
- Providing diabetes education and self-management support for people with Type 1, Type 2 and gestational diabetes.
- Conducting comprehensive assessments and developing individualised care plans.
- Supporting patients with insulin initiation and adjustment, glucose monitoring and diabetes technologies including CGM and insulin pumps.
- Delivering education to patients, families and healthcare professionals.
- Working collaboratively within multidisciplinary teams.
- Participating in health promotion and chronic disease management programs.
- Maintaining accurate clinical documentation and adhering to best practice guidelines.
About You
To be successful, you will have:
- Current unrestricted AHPRA registration as a Registered Nurse.
- Credentialled Diabetes Educator (CDE) status.
- Postgraduate qualification in Diabetes Education and Management.
- Current accreditation with the Australian Diabetes Educators Association (ADEA).
- Experience managing Type 1, Type 2 and gestational diabetes.
- Experience using Continuous Glucose Monitoring (CGM) and insulin pump technologies.
- Current Basic Life Support (BLS) certificate.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within multidisciplinary teams.
