Registered Nurse | Agency Aged Care & Community Care

We are working with a local community care organisation to appoint a dedicated and compassionate Care Partner to support older Australians living at home through Support at Home packages.

This role is ideal for experienced Community Enrolled/Registered Nurses seeking a rewarding transition into case management. You will build solid client relationships, lead new client onboarding, coordinate personalised care plans, and work closely with care teams to ensure high-quality services are delivered.

Key Responsibilities

- Develop and maintain trusting relationships with clients and families.
- Conduct onboarding, care reviews, and ongoing coordination using a wellness and reablement approach.
- Create tailored care plans and service agreements.




- Identify opportunities to expand services through client needs and community connections.
- Promote services to support growth and enhance outcomes.
- Work collaboratively with Caregivers and provide constructive feedback.

About You

- Current AHPRA registration
- Experience in aged care, case management, or Home Care Packages.
- Comprahensive knowledge of Support at Home framework.
- Experience in sales, growth, or business development is desirable.
- Strong interpersonal and communication skills.
- Confidence using Microsoft Office and CRM systems.
- Valid Australian Driver's Licence and willingness to travel across Brisbane North & Moreton Bay.
- Passion for empowering older Australians.
