Primary & Community Care Services in Ryde is seeking an experienced Implementation Scientist to lead Neighbourhood Health Hubs, applying implementation science, systems thinking and community-led approaches to design, test and sustain locally grounded care models.
You will work across clinical care, social care and community sectors, engaging residents, GPs, NGOs, councils and local leaders, to understand what works and why, with a focus on equity, inclusion and sustainable improvement.
