About the Role – Dementia Care and Support Coordinator
This is a part-time (4 days / 30.4 hours per week) fixed-term position until 30/06/2028.
The Dementia Care and Support Coordinator will lead the delivery of AMCS’s Dementia Respite Centre, providing high-quality, dementia-specific support services for people living with dementia and their carers. The role is responsible for coordinating programs, managing client care plans, engaging with community stakeholders, and supervising staff and volunteers to support positive outcomes for participants and families.
This role is primarily based across the City of Casey, with occasional attendance required at AMCS Head Office in Seddon (Melbourne’s Western Suburbs).
Key Responsibilities
- Coordinate the delivery and ongoing operations of the Dementia Respite Centre program.
- Conduct client assessments, care planning, reviews, and provide dementia-specific support to participants and carers.
- Develop and facilitate engaging group activities, carer education sessions, and wellbeing programs.
- Build strong relationships with clients, families, community groups, and key stakeholders.
- Supervise and support Respite Assistants and volunteers to deliver high-quality services.
- Monitor program outcomes, reporting requirements, client targets, and continuous improvement activities.
- Ensure compliance with aged care standards, health and safety requirements, and AMCS policies and procedures.
Key Selection Criteria
Essential
- Relevant qualifications in Dementia Care, Community Services, Leisure and Health, Aged Care, or a related field.
- Demonstrated experience supporting people living with dementia and their carers.
- Experience in care coordination, community engagement, and delivering person-centred support programs.
- Strong communication, interpersonal, and stakeholder engagement skills.
- Ability to lead, supervise, and support staff and volunteers.
- Ability to manage competing priorities and work independently.
- Current Victorian Driver’s Licence, First Aid and CPR Certificate.
- Police Check clearance (or willingness to obtain).
- Employee Working with Children's Check.
- Commitment to supporting culturally diverse communities and promoting inclusive care.
Desirable Criteria
- Experience working within aged care, dementia care, respite services, or community-based programs.
- Knowledge of dementia support practices, carer wellbeing, and person-centred approaches.
- Experience delivering group programs, education sessions, or community activities.
- Experience with care planning, reporting, and program evaluation.
- Experience working with multicultural communities and diverse client groups.
- Food Safety Handling Certificate.
