NSW Health’s Western Sydney Local Health District seeks a Chronic and Complex Senior Social Worker to provide advanced clinical social work in community health settings across Mount Druitt and Blacktown. This temporary part time role supports clients through grief, bereavement and end-of-life care.

You will mentor staff, lead complex case management, and collaborate with multidisciplinary teams to improve outcomes for people with chronic illness and palliative needs.
