What you’ll be doing in this role?

The Chronic Disease Care Coordinator - Lung Cancer Screening Navigator is responsible for providing participant-centred navigation across the screening pathway, from community awareness and outreach through to participant enrolment, screening completion and follow up of results. The position aims to improve equitable access to lung cancer screening by upporting eligible participants to overcome barriers to participation and assisting health services to implement effective referral pathways.

Your work will include:

- Identify potentially eligible NLCSP participants within Primary Care Clinics systems and ensure awareness of the program.
- Support participants to understand the benefits, risks and processes associated with lung cancer screening.
- Support community understanding of NLCSP and facilitate shared decision making about participation.
- Coordinate and monitor participant progression through pathway milestones including eligibility assessment, enrolment, imaging appointments and follow-up actions.




- Work closely with participants to identify and overcome barriers to NLCSP participation, including available community supports (e.g., transport, translators).
- Maintain database of participant information to monitor and track NLCSP participation for evaluation purposes.
- Contribute to project reporting, service improvement initiatives and evaluation activities

What you’ll need:

- Tertiary qualification in Nursing.
- AHPRA registration
- A current Victorian Drivers Licence.
- A current Working with Children Check (Employee)

- Extensive clinical experience working in a multi-disciplinary team.
- Experience working in a community setting
