Our opportunity
This role provides vital support to patients transitioning from hospital back into their homes and communities. You will play a key role in ensuring a smooth and supported discharge process by monitoring recovery, assisting with the organisation and attendance of specialist and allied health appointments, and providing supportive counselling, education, and debriefing following hospital stays.
The position also involves advocating for patients’ needs with recovery‑focused and community‑based organisations, helping to remove barriers to care and promote positive health outcomes. In addition, you will support community members seeking assistance to access Allied Health Services as outpatients, working within an early intervention model to enhance health, independence, and overall well being.
This is an opportunity to make a meaningful difference by supporting recovery, improving continuity of care, and strengthening connections between hospital, community, and outpatient services.
