Care Partner - Full Time (12-Month Maternity Leave Contract)
Hunter Health Care is seeking a compassionate and organized Care Partner to join our Home Care team. This is a full-time, 12-month maternity leave contract based in our Tomago office, the hours are 08:30-4:30 Monday to Friday. This position has potential for ongoing permanentemployment for the right candidate.

About the Role
The Care Partner plays a key role in ensuring our Home Care participants receive exceptional, person-centred support. Working closely with the Senior Care Partner, you will coordinate services, support our care staff, maintain accurate records, and liaise with participants, families and healthcare professionals to ensure high-quality care is delivered.
This role also provides administrative and rostering support to ensure the smooth day-to-day operation of our Home Care service.
Key Responsibilities
- Work collaboratively with the Senior Care Partner to deliver timely, person-centred services.
- Coordinate new Participant referrals and ensure services align with individual care plans.
- Compile and maintain accurate participants records within the TRACCS system.
- Liaise with Participants, families, healthcare professionals and external providers to ensure continuity of care.
- Advocate for Participants and respond to enquiries professionally and promptly.
- Support the Senior Care Partner in monitoring staff performance and service quality.
- Assist with organising staff meetings,
including preparing agendas and taking meeting minutes.
- Maintain accurate participant and staff documentation.
- Manage records, rosters and administrative processes using business systems.
- Complete general administrative duties, including filing, scanning, photocopying and data entry.
- Use the My Aged Care (MAC) and PRODA portals as required.
- Monitor the performance of external providers and suppliers.
- Assist with staff rostering.
- Undertake any other duties as directed.
About You
- Previous experience in aged care, community services, disability or NDIS administration.
- Excellent organisational and time management skills.
- Strong written and verbal communication skills.
- Outstanding customer service skills with a person-centred approach.
- Excellent problem-solving skills and a high level of attention to detail.
- The ability to work independently and collaboratively within a team.
- High-level computer literacy and confidence using digital systems.
- Experience using aged care management software (TRACCS experience highly regarded).
- Experience using PRODA and the My Aged Care portal.
- Experience with staff rostering and scheduling.
- A current Australian Driver Licence.
- Understanding of the Aged Care Quality Standards.
- Basic knowledge of human resources processes.
