We're looking for an organised, compassionate and proactive Service Delivery Coordinator to join our team.
About the Role
As our Service Delivery Coordinator, you'll play a key role in ensuring the smooth delivery of services across our participant base. You'll work closely with participants, families, support workers and the wider team to coordinate supports, manage rosters and ensure exceptional service delivery.
This is a fast-paced role that requires excellent communication skills, strong organisational skills and a genuine commitment to delivering outstanding participant outcomes.
Key Responsibilities
- Coordinate the day-to-day delivery of supports for NDIS participants, including participant onboarding and service commencements.
- Develop and maintain participant rosters, managing support worker availability and communicating roster changes to participants and staff.
- Match participants with suitable support workers based on their individual needs, goals and preferences.
- Build and maintain positive relationships with participants, families, Support Coordinators, Behaviour Support Practitioners and other key stakeholders.
- Coordinate services for participants with complex needs, ensuring supports align with participants' NDIS plans, goals and approved funding, escalating concerns where required.
- Respond promptly to service enquiries,
participant requests and changing support needs to ensure continuity of care.
- Monitor participant satisfaction, wellbeing and service quality, identifying opportunities for continuous improvement.
- Collaborate with support workers and the recruitment team to maintain appropriate staffing levels and support positive participant outcomes.
- Maintain accurate participant records and documentation in accordance with NDIS Practice Standards.
- Assist with incident reporting, safeguarding, compliance and quality assurance processes, escalating concerns promptly where required.
About You
You'll be someone who enjoys building relationships, solving problems and working in a supportive team environment.
You’ll also have:
- Previous experience in disability services, community services, aged care or a similar coordination role.
- A strong understanding of the NDIS and person-centred support.
- Excellent organisational and time management skills.
- Exceptional communication and interpersonal skills.
- The ability to manage competing priorities in a fast-paced environment.
- Strong administration and computer skills.li>A proactive, solutions-focused approach.
- The ability to work independently while contributing positively to a team.
- Current NDIS Worker Screening Clearance (or willingness to obtain).
- Current Working with Children Check (Blue Card).
- Current First Aid and CPR Certificate (or willingness to obtain).
- National Police Check.
- Full Australian working rights.
