Austin Health is recruiting a Social Worker Grade 3 Case Manager to join the new Older Adult Community Mental Health Team. This permanent, full time role focuses on psychosocial assessments and recovery-oriented planning in the community.

You will coordinate with families, carers, aged care providers and government services, advocating for consumer needs and sustainable community support. To succeed you will hold an approved social work qualification with AASW eligibility and at least two years’
