This is an exciting opportunity to join Ku-Ring-Gai Neighbourhood Centre Ltd (KNC) as a full-time Care Partner for Support at Home. In this key role, you will be responsible for overseeing the delivery of high-quality care services to our clients, ensuring their needs are met in a compassionate and efficient manner. The position is based in St Ives, NSW. Full time and part time positions available.

What you’ll be doing

- Developing and implementing care plans to meet the individual needs of each client
- Supervising and supporting a team of care workers to deliver exceptional client care
- Liaising with healthcare professionals, families and other stakeholders to coordinate holistic care
- Monitoring and evaluating client progress, making adjustments to care plans as needed
- Ensuring compliance with all relevant healthcare regulations and standards
- Implementing quality improvement initiatives to continuously enhance our clinical services
- Contributing to the overall strategy and growth of the organisation

What we’re looking for

- Bachelor's degree in social or Community or a related field
- 2+ years' experience in a care management role, preferably in the community healthcare sector
- Excellent communication and interpersonal skills, with the ability to collaborate effectively with diverse stakeholders




- A background in working with clients who have complex needs
- Solid clinical expertise and a deep understanding of best practices in client-centred care
- Proven track record of leading and developing high-performing teams
- Exceptional problem-solving and decision-making abilities
- Commitment to upholding ethical standards and providing compassionate care
