Anglicare Sydney is seeking a Health & Wellness Coordinator for Cobbitty Village. The role focuses on supporting residents’ health, wellbeing and independence, working with families and healthcare professionals to connect services and activities.

The position is part time and on-site, primarily Mondays and Wednesdays, with duties spanning wellbeing plans, My Aged Care processes and community engagement. You will coordinate referrals, maintain records in CRM, and collaborate with Village Managers
