The Aboriginal Tenants at Risk Transitional Support role provides case management to community members who are successful in nomination to be tenanted to our Aboriginal specific THM’s supported by Wathaurong.
With a solid focus on strengths-based practice, the case manager works alongside the community members to strengthen their capacity to establish and maintain transitional housing tenancies, while addressing support needs which contributed to their homelessness and working towards exit plan to long-term housing. The role will also support and provide outreach to community members in crisis and emergency accommodation along with hold a number of cases within the homelessness entry point team space where needs cannot be met within scope of IAP response.
The role works closely alongside and in co-ordination with both the Aboriginal Tenants at Risk, Transitional Support programs and Aboriginal Private Rental Assistance Program Broker.
