An exciting opportunity is available for an enthusiastic NDIS Support Coordinator to join MS Plus Support Coordination Team, coordinating participants in our Hawthorn East area.
We are seeking a positive, well-organised individual with a solid understanding and experience in NDIS processes who is passionate about making a difference.
Key Responsibilities
Work with NDIS participants, families, and other stakeholders to implement, monitor and review NDIS plans;
Support implementation of all supports in the plan, including informal, mainstream and community, as well as funded supports;
Connecting participants with both formal and informal supports to improve their quality of life;
Ethical, customer focused practice to provide support to the participants to achieve their goals;
Identify, refer to, and coordinate appropriate service providers and supports for participants, including coordinating the implementation of service agreements;
Provide ongoing support and guidance to participants and their support network to ensure that goals are achieved and services are delivered in accordance with the participant's NDIS plan and budget.
Complete and submit support coordination reports to the NDIA, including implementation reports, progress reports, and end of plan reports.
Skills and Experience
Effective time management and organisational skills, ability to prioritise work and meet deadlines;
An understanding of the NDIS and its impact on the lives of people with a disability;
An ability to assess and respond to individual needs;
The ability to communicate effectively with a diverse range of people tobuild trust and develop rapport;
Be able to work independently and as part of a team, and to provide guidance and support to less experienced staff as required.
Proactive approach in assisting participants to manage their NDIS funds to ensure issues are identified and addressed early.

If successful you will need to
Complete a National Police Check (and possibly an International Police Check);
Provide a NDIS Worker Screening Check;
Provide a Working with Children's Check or vulnerable people check,
Have a valid drivers licence.
