Gidgee Healing in Mount Isa seeks an Elder Care Connector to support Aboriginal and Torres Strait Islander Elders in accessing culturally appropriate aged care services.
This role involves community outreach, engagement with Elders and families, and assisting with My Aged Care processes.
You will advocate for clients, identify service gaps, and collaborate with the Aged Care Support Coordinator and internal teams.
Travel to remote areas and a trauma-informed approach are required.
