As a Program Manager, you will:

- Build and maintain solid referral networks (GPs, hospitals, allied health, support coordinators and community organisations)
- Identify and pursue new referral opportunities within the local community
- Support growth targets for Support at Home and NDIS services
- Conduct client intake, home visits and assessments
- Develop person-centered care plans aligned with funding guidelines
- Coordinate and monitor service delivery to ensure quality and continuity
- Monitor client budgets and ensure services remain within funding guidelines
- Act as the key contact for clients and families





You will play a key role in strengthening Holicare's community presence while ensuring high-quality, compliant and person-centered service delivery.

About You

- Experience in aged care, disability or community services
- Strong knowledge of Support at Home and/or NDIS funding models
- Strong case planning and case management skills
- Confidence in building professional networks and promoting services
- Excellent communication and stakeholder engagement skills
- Ability to manage KPIs and competing priorities
- Current driver's license and willingness to travel locally
- Experience with systems (e.g. Nightingale, EziPlan, Share Point)
- Understanding of quality and safeguarding frameworks

Qualifications in Community Services, Nursing, Social Work Case Management, or related fields are highly regarded
