We are looking for an experienced Clinical Care Coordinator to provide expert resident assessment, care planning, quality care consultations and build relations with our residents, their families and staff in our care community.
Brimlea Aged Care is located in quiet Murrumbeena, minutes by foot from two train stations and a short walk from the iconic Chadstone shopping centre.

Clinical Care Coordinator - Key Responsibilities
Clinical Leadership: Provide expert clinical leadership, coaching, support and education to nursing and care teams to promote a culture of safe, person‑centred care.
Resident Admissions: Conduct resident admissions with warm and welcoming approach to support transition into residential care, initiate relevant assessments and charting and monitor completion of the same.
Clinical Governance: Provide oversight of clinical practices, ensuring Registered Nurses (RNs) and Enrolled Nurses (ENs) deliver care in line with organisational policies, best practice, and regulatory requirements.
Care Coordination: Undertake complex care planning, resident assessments, and case conferencing to support individualised care needs as they fall due and as needed.
Acuity & Risk Management: Monitor resident health status, respond to clinical deterioration, and coordinate timely interventions including GP and allied health reviews.
Quality & Compliance:
Ensure care delivery aligns with the Strengthened Aged Care Quality Standards and support continuous improvement and accreditation readiness.
Funding (AN-ACC): Coordinate and complete AN-ACC assessments and reappraisals in accordance with funding requirements.
Incident & Risk Management: Oversee clinical incident reporting, investigation, and compliance with the Reliable Incident Response Scheme (SIRS).
Operational Support: Conduct clinical audits, monitor KPIs, and contribute to workforce capability and performance development.
Documentation & Systems: Ensure accurate and timely clinical documentation in electronic care management systems.
About you:
Registered Nurse with current registration with Australian Health Practitioner Regulation Agency (AHPRA)
Demonstrated experience in clinical leadership within aged care
Sound knowledge of the Aged Care Quality Standards and relevant legislation, including the Aged Care Act 2024
Working knowledge of the AN-ACC funding model
Ability to oversee clinical governance, including medication management, wound care, infection control, and incident management
Experience in leading, mentoring, and supporting clinical teams
Commitment to continuous quality improvement and resident‑centred care
Current National Police Check, vaccinations, and compliance documentation
