- Mental Health/AOD Clinician permanent part time for Mental Health Statewide Partnering Provider- Hamilton Centre

About the Hamilton Centre

Austin Health and Goulburn Valley Health Partner Provider Service follows a shared, stepped care model, with the clientremainingunder the direct care of the referring healthcare provider and/or team throughout the duration of service. The Austin Health and Goulburn Valley Health Partner Provider Service further supports a collaborative approach to client care, to enhance the capacity and response of referring healthcare providers and teams, clientsand carers to better manage substance use and addiction issues within the eastern population.

This service has been established to provide primary and secondary consultation to people living with co-occurring disorders, who are engaged with AOD services, primary and secondary care services, local mental health and wellbeing services and area mental health services in the northeast region, as its core activities

About the role

The Hamilton Centre RPN3 will work collaboratively with the Program Manager, Nurse Practitioner and Consultants working with consumers with co-occurring mental health and substance use or addiction needs, including therapeutic and clinical supports. Support virtual appointments where thisexpeditesand supports access to care without reducing clinical quality.





The Hamilton Centre provides primary and secondary addiction medicine assessments, along with capacity building for all area mental health and well-being services, local mental health services and AOD services across Loddon Mallee (Bendigo Health and Mildura Base Hospitals), Hume (Goulburn Valley Health and Albury Wodonga Health) and Northeast Metro (Austin Health). The Hamilton Centre RPN3 assists in referral intake, coordinating consumer flow, delivering education sessions to mental health and AOD teams and provide support to stakeholders for integrated care between mental health and AOD services.

About you

You will be able toestablishlinkages and liaison with community partnering services and all stakeholders involved. You will have the ability to accept and triage referrals from our regional partners via Turning Point and to coordinate referrals appropriately.

To be successful in this roleyou’llbring with you:

- Registered Nurse registered with Australian Health Practitioners Regulation Agency
- Knowledge of the Mental Health and Wellbeing Act 2022 and other relevant legislation.
- Relevant professional qualification in Mental Health nursing
- Demonstrated knowledge of recovery and collaborative clinical practice
- A consumer centered approach to care
- A flexible, innovative team-oriented approach to service delivery




- Proficiencyin information technology systems and applications (Office 365, search engines, databases, etc.)
