At Health and Community Services (HCS), we are seeking a skilled and motivated Occupational Therapist to join our growing multidisciplinary team.
In this role, you will support NDIS participants to maximise their independence, safety, and quality of life through evidence-based, person-centred interventions. You will work across a diverse caseload, delivering meaningful outcomes in both community and home environments.
Key Responsibilities
Conduct comprehensive functional and capacity assessments to identify client needs
Complete NDIS Home & Living Assessments, supporting appropriate housing and support recommendations
Develop and implement individualised, evidence-based intervention plans
Support clients to improve mobility, self-care, and participation in daily activities
Recommend and facilitate assistive technology and home modifications
Collaborate closely with support coordinators, behaviour support practitioners, and allied health professionals
Maintain accurate, compliant clinical documentation and reporting
Contribute to continuous improvement and best practice service delivery
About You
You are a passionate and clinically confident Occupational Therapist who thrives in a community-based setting.
Bachelor’s or Master’s degree in Occupational Therapy
Current registration with Australian Health Practitioner Regulation Agency
Experience working within the NDIS framework (preferred)
Strong clinical reasoning and assessment skills
Excellent communication and stakeholder engagement abilities
Ability to work independently and manage a diverse caseload
Requirements
Current Driver’s Licence and reliable vehicle and insurance
Working with Children Check
NDIS Worker Screening Check (or willingness to obtain one — support will be provided).
