About the Role As a Care Partner – Support at Home

you’ll support older Australians receiving CHSP or Support at Home funding, helping them remain connected, confident, and in control of their lives.

This role is about more than coordinating services — it’s about building meaningful relationships, understanding each person’s goals, and supporting them to live independently with dignity and choice.

You'll be responsible for:

- Providing person-centred case management and ongoing support
- Developing meaningful care plans aligned with individual goals and needs
- Coordinating services and working collaboratively with clients, families, and service providers
- Supporting clients to maintain independence, wellbeing, and quality of life
- Identifying opportunities to improve outcomes and ensure clients receive the right support at the right time

You’ll be someone who notices the small things, celebrates progress, and helps clients feel empowered to live life their way — safely, confidently, and in the comfort of their own home.

About You You'll thrive in this role if you have:

- Minimum 3 years’ experience in case management and/or aged care, home care, or a related field




- Strong communication and interpersonal skills, with the ability to build rapport with a wide range of stakeholders
- The ability to work independently, exercise sound judgement, and manage competing priorities in a quick-paced environment
- Experience with budgeting, funds management, and service coordination
- A demonstrated understanding of risk management, including hazard identification and incident response
