Community Health Access Manager - Allied Health
Some people need allied health care and can't access it. This role exists to change that.
Optimum Health Solutions is establishing a community social responsibility program — a formal pathway for people in genuine financial hardship to access assessment and treatment services across our network that they would otherwise be unable to afford. We need a registered allied health clinician to build it and lead it.
As Community Health Access Manager, you will develop the intake and triage systems, build the referral partnerships, and coordinate access to care for people referred into the program through community organisations, social workers, GPs, hospitals, and schools. You will assess need, determine eligibility, match people to the right Optimum service, and remove the barriers that typically stop vulnerable people from getting the help they need.
You will also carry a clinical caseload of program participants, staying connected to the people this program exists to serve.
This is a role for a clinician who is motivated by equity of access, not just service volume. Someone who knows how to navigate a complex system on behalf of someone who can't do it themselves.
What you'll be doing
- Establishing Optimum's community social responsibility program from the ground up
- Developing intake, triage, and referral processes for people in financial hardship
- Assessing need and coordinating access to Optimum services across NSW and Tasmania
- Building referral partnerships with community organisations, GPs, hospitals, and social services
- Maintaining a clinical caseload of program participants within your own discipline
- Reporting on program activity, outcomes, and partnership development to the Managing Director
What you'll bring
- Tertiary qualification in an allied health discipline with current AHPRA registration
- 4+ years post-graduate experience with exposure to community health, social services, or working with vulnerable populations
- Experience in case coordination, triage, or referral management
- Solid understanding of the barriers that prevent people in hardship from accessing allied health
- Excellent interpersonal skills and the ability to build trust across diverse and difficult circumstances
- Current Working with Children Check and driver's licence
