Proactive Health is seeking a Clinical Social Worker and Care Coordinator to support veterans and Medicare clients from our Fortitude Valley head office in Brisbane. You will deliver telehealth care, run intake assessments, create care plans and lead 90-day reviews with an allied health team.

You will coordinate referrals across mental health, physiotherapy, podiatry and more, document in Halaxy, and participate in GP-led case conferences.
