As Pathways to Good Health Navigator, you will play a vital role in improving health and wellbeing outcomes for Aboriginal children and young people entering care. Working closely with children, young people, carers, families, health providers and community partners, you will provide culturally safe health navigation, advocacy and service coordination support. You will help identify health and wellbeing needs, facilitate access to assessments and services, develop individual support pathways, and strengthen connections to culture, community and holistic wellbeing supports.

About the Role
Reporting to the Specialist Support Senior Team Leader, the Pathways to Good Health Navigator provides culturally safe health navigation, advocacy and service coordination support to Aboriginal children and young people entering care. The role works closely with children, young people, carers, families, health providers, Aboriginal Community Controlled Organisations and other key stakeholders to facilitate timely access to health assessments, wellbeing services and ongoing supports.
A key focus of this role is supporting Aboriginal children and young people to overcome barriers to accessing health, wellbeing and community services while promoting cultural connection, self-determination and holistic wellbeing. Through effective relationship building, care coordination and advocacy, the Pathways to Good Health Navigator helps ensure children and young people receive the support they need to achieve positive health and wellbeing outcomes.
Key Responsibilities
As the Pathways to Good Health Navigator, you will:
- Provide culturally safe health navigation and support to Aboriginal children and young people entering care, assisting them to access timely health assessments, wellbeing services, and ongoing supports that promote positive health outcomes.
- Support the development and implementation of Pathways to Good Health plans, working with children, young people, carers and families to identify health and wellbeing needs, establish goals and strengthen access to appropriate supports.
- Facilitate referrals and service access, advocating on behalf of children and young people to reduce barriers and improve engagement with culturally safe health, wellbeing, community and specialist services.
- Build strong and respectful relationships with children, young people, carers and families, ensuring practice is strengths-based, trauma-informed, culturally responsive and focused on self-determination.
- Work collaboratively with key stakeholders and service providers, including Aboriginal Community Controlled Organisations, health services, child and family services and community partners to support positive outcomes for children and young people.
- Promote Social and Emotional Wellbeing, supporting children and young people to strengthen cultural identity, community connection and holistic wellbeing through culturally informed practice.
- Maintain accurate case notes, records and reporting, ensuring all documentation and information management requirements are completed in accordance with organisational, funding and legislative obligations.
- Monitor client progress and wellbeing outcomes, responding to emerging needs and supporting appropriate adjustments to services and supports where required.
- Contribute to program outcomes and service improvement activities, providing feedback, identifying opportunities for improvement and supporting quality service delivery.
- Promote culturally safe and community-controlled practice, ensuring all work reflects Aboriginal Community Controlled principles and contributes to positive outcomes for Aboriginal children, young people and families.
About You You are a passionate and organised professional with experience working with Aboriginal and Torres Strait Islander children, young people, families and communities. You have a strong understanding of culturally safe practice, Social and Emotional Wellbeing principles, and the ability to build trusted relationships that support positive health and wellbeing outcomes.
You are a strong communicator who can work effectively with families, carers and service providers, while managing competing priorities and maintaining accurate records. You are committed to strengths-based, trauma-informed practice and are motivated by the opportunity to improve outcomes for Aboriginal children and young people within an Aboriginal Community Controlled Organisation.
Key Selection Criteria
Essential:
- Certificate IV in Community Services, Aboriginal and/or Torres Strait Islander Primary Health Care, Youth Work, Mental Health, Alcohol and Other Drugs, Child, Youth and Family Intervention, or a related discipline, together with relevant experience; or an equivalent combination of qualifications, skills, knowledge and demonstrated experience.
- Demonstrated ability to work with a degree of independence, exercise sound judgement and manage a diverse workload within established program guidelines.
- Demonstrated experience working with Aboriginal and Torres Strait Islander children, young people, families and communities.
- Strong understanding of Aboriginal culture, self-determination, Social and Emotional Wellbeing principles and culturally safe practice.
- Demonstrated ability to engage effectively with children, young people, carers and families using a strengths-based and trauma-informed approach.
- Experience supporting clients to access health, wellbeing, community and support services through navigation, advocacy and referral processes.
- Well-developed interpersonal, communication and relationship-building skills,
including the ability to work effectively with a broad range of stakeholders.
- Demonstrated ability to build and maintain effective working relationships with Aboriginal Community Controlled Organisations, health services and community service providers.
- Ability to identify barriers to service access and support children, young people and carers to engage with appropriate services and supports.
- Demonstrated organisational skills, including the ability to manage competing priorities, maintain accurate records and meet reporting requirements.
- Experience using client management systems and Microsoft Office applications, including Word, Excel and Outlook.
- Demonstrated commitment to culturally safe, child-centred and family-focused practice.
Desirable:
- Identify as an Aboriginal and/or Torres Strait Islander.
- Experience working within an Aboriginal Community Controlled Organisation (ACCO).
- Knowledge of child and family services, out-of-home care, health and wellbeing systems and referral pathways.
- Experience working with Aboriginal children and young people entering care or involved with the child protection system.
- Understanding of the Victorian Aboriginal Child and Family Service sector and relevant legislation, policies and frameworks.
- Experience in community engagement, health promotion, wellbeing programs or care navigation services.
- Knowledge of Social and Emotional Wellbeing frameworks and holistic approaches to Aboriginal health.
- Current First Aid Certificate or willingness to obtain.
