South Eastern Sydney Local Health District in Sydney CBD NSW is seeking a Clinical Assessor to perform complex aged care assessments using the Integrated Assessment Tool (IAT) and develop support plans for older people and their carers.

You will work within NSW Health guidelines, liaise with a multi-disciplinary team, and apply risk management and safety principles while travelling as required.
