Working part time for 20 hours a week (In time it will be full time as we grow) at our office in Gosford/Maitland , you will be someone who can be relied upon to do what’s best for the person benefiting from your service. You will be someone who is attentive, and who will be proactive in supporting, noticing and reporting changes in physical health and emotional well being. You will be 100% supported by an experienced team who share your passion, dedication, resilience and problem-solving ability in the face of the inevitable challenges that can present themselves.
About the Role
Manage a caseload of clients with complex needs through client meetings & assessment, referral, review and goal planning
Managing client funds and budgets
Building strong internal & external relationships including client's families, allied health providers and support workers
Identify and grow new business opportunities through promotions/expo’s/networking
Provide appropriate support to culturally & linguistically diverse clients
Ensure compliance with relevant legislation
Part of this role is to engage with other referral partners (clinical nursing/allied health/my aged care etc)
Preferred Attributes
Previous experience in Aged Care or Community Services in Care Management is ideal but not essential
Tertiary qualifications (Diploma in Community Services or Cert. IV in Aged Care) is ideal but not essential
Be a natural problem solver and creative thinker with the ability to get the very most out of the package for your client.
Contribute to ideas and continuous improvement for the company's performance growth.
Demonstrated understanding of the needs of older people and their carers
Resilient and able to work effectively under pressure and in an ever changing environment
High attention to detail with focus on achieving positive consumer outcomes
Demonstrated experience in case management, including managing government-funded care packages and developing budgets
Demonstrated ability to create and implement consumer-directed services
Well-developed communication (written and verbal), interpersonal, problem solving, client database management and time management skills
Team oriented with an open and collaborative work style
Current driver’s licence and insured vehicle
National Police Clearance/NDIS Check
