Job Title: Care Partner: Care Management – Support at Home Program

(This job position is community based and not a job position based in the Violet Town Bush Nursing Home)

Honeysuckle Regional Health Homecare Service (HSRH) is a not for profit community organisation and approved provider of the Support at Home Program based locally at the Violet Town Bush Nursing Centre.

HSRH has been supporting older Australians in the community by providing access to services, assistive technology and home modifications. Our goal is to work with participants and their families to develop a care plan to stay healthy, active, and connected to their community and continue to live well at home.

We’re looking for a welcoming and caring ‘Care Partner’ to join our team, who is passionate about aged care and offering an effective and prompt individual personalised local service.

Care partner key responsibilities

· Deliver relationship-based, person-centred, coordinated care that supports each participant’s goals and preferences.

· Build strong relationships with participants, families and associated providers.

· Monitor and responds to changes in health, circumstances and wellbeing.

· Promote independence through wellness and reablement approaches.

· Support the participant (and their carers and registered supporters) to navigate aged care systems and services.

· Ensure services are delivered in a culturally safe and timely manner.

· Maintain clear and respectful communication that considers the participant’s communication preferences.

· Document care accurately to ensure transparency and continuity of support.

· Conduct assessments, referrals, and regular reviews.





· Monitor the SAH Package Budget and ensure clients understand service fees and client contributions.

· Work in collaboration with the Support at Home Program Manager and the Administrative Team to facilitate, develop and promote our Vision, Mission and Values.

· Ensure compliance with all relevant legislative and industry standards.

Care partners are critical to the delivery of quality care and services. It is important the care partner establishes a trusting relationship with the participant, their registered supporter and their family members to gain a full understanding of the supports required.

About You:

You’re a welcoming, caring, genuine, and proactive individual with a passion for aged care and supporting older people to live well at home. You are confident working independently and in a team environment and have a strong ability to manage and prioritise a caseload.

You will bring:

· Minimum 2 years’ experience in a similar Care Partner/ Case Manager role in the Support at Home or

Home Care Package Program.

· Use of eTOOLS: eSAH highly preferred: Support at Home Software.

· Minimum Diploma in Community Services or Case Management.

· Exceptional interpersonal, communication, negotiation, and time management skills.

· Strong IT skills and understanding of The Strengthened Aged Care Quality Standards and Support at

Home Program.

· Must be able to multitask.

· Be willing to travel to participants around Violet Town, Euroa, Benalla and districts with access to a company vehicle.

Requirements:

· Current Victorian Driver’s Licence

· Current Police Check (within the last 12 months)

· Must be an Australian Citizen or Permanent Resident
