• Growth Support Contribute to service growth and community engagement initiatives Promote services to local hospitals, ACAT, and community partners What you need to be successful in this role You are a compassionate, organised, and proactive professional who is passionate about delivering high-quality aged care.
Essential Criteria: Certificate IV in Individual Support (Ageing) Demonstrated experience in community aged care (SAH & CHSP) Strong experience in client assessment and care planning Proven leadership and team development skills Excellent time management and problem-solving ability High level of communication and interpersonal skills Experience with electronic care systems and Microsoft 365 Sound understanding of Aged Care Quality Standards and compliance Additional Requirements: Current National Police Clearance (within 12 months) Ability to participate in an on-call roster Previous case management experience (highly regarded) COVID-19 and Influenza vaccinations (recommended) This role is employed on a salaried contract, and you will be required to participate in an on-call roster one week in each four weeks, including the weekend. When undertaking on-call you will be provided with a day off lieu providing you with flexibility.
