Pearl Home Care Bunbury South West is a locally owned and operated home care provider supporting older people across Bunbury and surrounds. We're growing, and we're looking for a Care Partner to join our clinical team.

What you'll be doing

- Completing initial and ongoing client assessments in the home

- Developing, reviewing and updating care plans and risk assessments

- Acting as the named care partner for a caseload of Support at Home clients - the consistent point of contact for the client, their family and their representatives

- Monitoring clinical needs

- Coordinating with GPs, allied health, hospitals and My Aged Care

- Supporting, briefing and mentoring our support workers on each client's needs

- Recording care management activity and keeping documentation compliant with the Aged Care Quality Standards

- Reviewing budgets and service mixes with clients so their funding is used well

What we're looking for

- A registered nurse, preferably with experience in community and aged care
- A working understanding of Support at Home,



or the confidence and willingness to learn it quickly

- Genuinely good with people - clients, families, GPs and support workers all have to trust you

- Clear, accurate documentation habits

- Current WA driver's licence and reliable vehicle

- National Police Clearance (or willingness to obtain)

- Current First Aid and CPR
