NDIS Support Coordinator (Permanent Part-Time or Full time)

We are seeking an experienced and passionate NDIS Support Coordinator to join our team on a permanent part-time or full-time basis.

About the Role:
- Assist participants to understand and implement their NDIS plans.
- Connect participants with appropriate supports and community services.
- Build participants' capacity to achieve their goals and increase independence.
- Coordinate services and advocate for participants when required.
- Maintain accurate case notes and documentation in line with NDIS Practice Standards.
- Develop and maintain strong relationships with participants, families, service providers, and key stakeholders.
To be successful in this role, you must have:
- Minimum 2 years' experience working as an NDIS Support Coordinator.
- Relevant TAFE or tertiary qualification in Community Services, Social Work, Disability, Psychology, or a related field.
- Current Working With -Children Check (WWCC).
- Current National Police Check.
- Current NDIS Worker Screening Check.
- Excellent communication, organisation, and problem-solving skills.
- Strong knowledge of the NDIS, its legislation, and the local service network.
- Current Australian Driver's Licence and reliable vehicle
