Aged Care, Disability & Palliative Care Division About the role The Transitional Aged Care Program (TACP) within Western NSW Local Health District is seeking a Registered Nurse to join a dynamic, multidisciplinary team committed to supporting older people as they regain independence following a hospital stay.
This role contributes to delivering a tailored package of clinical and non-clinical services designed to enhance physical and cognitive functioning and reduce the risk of hospital readmission or early entry into residential aged care. The successful candidate will help promote restorative care, reablement and wellness, ensuring older people receive individualised support that enables them to regain independence and remain safely in their own homes. As part of the broader Aged Care Division, this role contributes to a service that spans the entire Local Health District and plays a vital role in helping older people remain supported in their own homes.
