What you'll be doing

The ASET Registered Nurse undertakes the responsibility and accountability for the delivery of safe and efficient patient focussed health care within allocated resources and clinical areas.

- To provide processes and support to limit hospitalisation for the aged care patient in cooperation with other health professionals, family and community based services.

For ensuring all older people admitted to the Emergency Department are screened and identified if appropriate as an aged care client requiring a comprehensive assessment (including cognitive and behavioural assessment) as soon as practical after their presentation.

To liaise and work with Aged Care Emergency Team to identify older people at risk in the community and proactively develop management plans to maintain safety and independence.
