Our client, Metro South Health, is seeking an experienced Aboriginal and/or Torres Strait Islander Registered Nurse to join the Southern Queensland Centre of Excellence.
As the Clinical Nurse within the HOMES, Holistic Outreach Model of Engagement and Support, Child and Adolescent team, you will play an important role in improving health outcomes for Aboriginal and Torres Strait Islander children, young people and their families.
You will provide advanced clinical care, case management and care coordination through a strengths-based and culturally responsive approach.
About the role
As the Clinical Nurse, you will deliver advanced nursing care and holistic case management to Aboriginal and Torres Strait Islander children, young people and families living with complex health needs.
In this role, you will:
- Coordinate and deliver advanced nursing care and case management
- Provide home visits and outreach services across Metro South Health
- Build trusted relationships with children, young people and families
- Develop holistic care plans that support self-management and wellbeing
- Coordinate care across health, community and social support services
- Promote culturally safe and evidence-based nursing practice
About you
Our client is looking for an experienced Registered Nurse who is passionate about improving the health and wellbeing of Aboriginal and Torres Strait Islander children, young people and families.
You will also bring
- Demonstrated experience managing complex chronic conditions
- Strong case management and care coordination skills
- Experience working within Aboriginal and Torres Strait Islander health
- Excellent communication and relationship-building capabilities
- A cooperative approach focused on health equity and community partnerships

About the Southern Queensland Centre of Excellence
The Southern Queensland Centre of Excellence for Aboriginal and Torres Strait Islander Primary Health Care is based at Inala and forms part of the Inala Indigenous Health Service.
Its services include
- General practice, nursing and allied health clinical services

- Integrated mental health and substance use services
- Culturally responsive support for Aboriginal and Torres Strait Islander people
- Outreach delivered in a person’s home or another location chosen with them
