Your new role:

As a Clinical Nurse within the PAH Community Care Team, you will use your specialist mental health nursing expertise to provide recovery-oriented care to adults experiencing severe and enduring mental illness, while delivering clinical leadership within a multidisciplinary community setting.

Deliver comprehensive mental health assessments, evidence-based interventions, care coordination and recovery-focused case management for consumers with complex mental health needs.

Provide specialist clinical leadership and expert decision-making to support protected, person-centred care, including risk assessment, physical health monitoring and discharge planning.

Work collaboratively with consumers, families, carers and multidisciplinary teams to develop and implement individualised care plans that promote recovery and community participation.

About you:

You are an experienced mental health nurse with a passion for recovery-oriented practice and delivering high-quality care to consumers with complex mental health needs.

Demonstrated experience in mental health nursing, with the clinical expertise to assess, manage and support adults experiencing severe and enduring mental illness within a community or acute mental health setting.

Current registration as a Registered Nurse with the Nursing and Midwifery Board of Australia (NMBA), with postgraduate mental health qualifications and/or Mental Health Nurse Credentialing viewed favourably.

Strong clinical judgement, communication and relationship-building skills, with the ability to work autonomously while collaborating effectively within a multidisciplinary team.
