Role Description The Community Support Worker role is a contract, on-site position based in the Greater Brisbane Area. In this role, the worker provides direct support to people with disabilities, assisting with daily activities, community participation, and social engagement.

Responsibilities include helping individuals access local services, facilitating inclusion in community programs, and offering emotional support. The role also involves collaborating with families, service providers, and other stakeholders to coordinate care and advocate for client needs.

Qualifications

Candidates should possess strong interpersonal skills and the ability to build trust-based, supportive relationships.

Candidates should possess community support and community health experience, with a focus on disability and inclusion.

Candidates should possess clear and respectful communication skills, both verbal and written.

Candidates should possess case management capabilities, including documentation, coordination of services, and follow-up.

Relevant qualifications in community services, social work, disability support, or a related field are beneficial.

Knowledge of local community resources and supports in the Greater Brisbane Area is an advantage.

Ability to work collaboratively within a multidisciplinary team and follow person-centered care principles.

Current working with vulnerable people checks, relevant registrations, and valid driver’s license may be required.
