What you'll be doing The Aboriginal Care Coordinator (ACC) will work as part of the Coordination of Care Pilot Program currently being conducted by the Cancer Institute.
The Program aims to reduce the challenges that Aboriginal people may experience in accessing and engaging with cancer treatment and support services, including cultural, social, and practical barriers that often impede access to the health care system and specifically within Cancer Services.
About You
Applicants must be of Australian Aboriginal and/or Torres Strait Island descent. Aboriginality is a genuine occupational qualification as authorised under S21 of Anti-Discrimination Act 1977.
You are a dedicated Aboriginal health professional with relevant qualifications and extensive experience supporting Aboriginal people, families and communities to achieve positive health outcomes. Drawing on your strong understanding of the health and social issues impacting Aboriginal communities, you provide culturally protected, respectful and client-focused care while building trust, strengthening engagement and advocating for the needs of Aboriginal patients across a range of healthcare settings.
You are a skilled communicator with the ability to develop strong relationships with patients, families, carers, community organisations and healthcare providers. Your experience in advocacy,
liaison and stakeholder engagement enables you to support culturally appropriate service delivery and improve access to healthcare services for Aboriginal people. Highly organised and outcomes-focused, you effectively manage competing priorities, meet deadlines and coordinate projects while maintaining a strong commitment to quality service delivery.
You work confidently both independently and as part of a multidisciplinary team, demonstrating leadership, initiative and the ability to motivate and support others. With strong project implementation experience and proficiency in Microsoft Office applications, you contribute to the successful delivery of programs and initiatives that improve community health outcomes. Holding a current unrestricted NSW driver's licence and a willingness to travel, you are committed to providing flexible and responsive support to Aboriginal communities across diverse locations.
