About the Role:

- We have an exciting opportunity for a Nurse Practitioner to join our Monash Medical Centre Nephrology Team on a Full-Time (Fixed-Term) basis.

- Full-Time, Fixed-Term position (2 years)

The Nephrology Nurse Practitioner Candidate (NNPC) is an advanced practice nurse working toward Nurse Practitioner endorsement, delivering expert, evidence-based care across the chronickidney disease (CKD) continuum (Stages 3B–5).

This role is integral to the multidisciplinary Nephrology team, supporting the healthcare needs of patients across inpatient,outpatientand community settings in the south-east corridor of metropolitan Melbourne and regional Victoria. The position may be based across multiple Monash Health sites in line with service requirements.

The NNPC practises with increasing autonomy and accountability, working in close collaboration with endorsed Nurse Practitioners and Nephrology Medical Specialists to deliver comprehensive episodes of care.

Core Clinical Areas:

- Nephrology Nurse Practitioner-aligned clinics
- Renal outpatient services




- Quality improvement and service development
- Kidney transplantation
- Kidney failure and diabetes management

About Us:

Monash Health is the quaternary provider of nephrology services across south-east metropolitan Melbourne and regional Victoria. The serviceoperateswithin a hub-and-spoke dialysis model, with the Clayton campus as the primary hub, and functions as a Level 6 service under the Victorian Renal Delineation Framework.

Services include:

- Chronic kidney disease prevention and management
- Dialysis services (inpatient,homeand satellite models)
- Kidney and pancreas transplantation
- Research, education and training programs

About You:

You are an experienced and motivated advanced practice nurse committed to progressing toward Nurse Practitioner endorsement.

You thrive in a dynamic clinical environment anddemonstratea strong commitment to evidence-based, person-centred care, clinical leadership, and service innovation.

You will be supported to develop your scope of practice and contribute to the ongoing advancement of nephrology services at Monash Health

What You Need:

- Enrolled in, or eligible for enrolment in, a Master of Nurse Practitioner program
- Current AHPRA registration as a Registered Nurse
- Minimum 4 years post-registration experience, including:
- ≥3 years in nephrology nursing
- Experience in advanced practice nursing roles
- Demonstrated advanced clinical reasoning, decision-making and leadership capability
- Evidence of engagement in quality improvement and/or research activities
- Commitment to achieving Nurse Practitioner endorsement within the expectedtimeframe
