What you will do:





- Provide guidance and support to young people leaving care, helping them transition to independent living.
- Develop and implement personalised support plans, considering the unique needs of each individual.
- Collaborate with external agencies and other professionals to ensure comprehensive care and support.
- Conduct regular reviews to adjust plans and services to meet the evolving needs of the young people in your care.
- Act as an advocate, ensuring that young people have access to the services and opportunities they deserve.

Requirements:

- A degree or equivalent qualification in social work.
- Current registration with Social Work England.
- Significant frontline experience in UK Social Services, working as a qualified Social Worker.
