Key Responsibilities

- Assist with CEO and team to work on Support at Home application documents, go through accreditation audit process
- Contribute to the oversight of clinical governance, risk management, and quality improvement initiatives
- Review clinical incidents, trends, audit outcomes, and consumer feedback relating to dementia care
- Support development of dementia-friendly and person-centred care models
- Participate in committee meetings and strategic governance discussions
- Support organisational compliance with Aged Care Quality Standards and clinical governance obligations




- Promote continuous improvement and positive consumer outcomes across home care services

What We Are Looking For

To be successful in this role, you will have:

- Experience working within or demonstrated understanding of the Strengthened Aged Care Quality Standards framework
- Relevant tertiary qualifications in Nursing, Medicine, Allied Health, Psychology, Public Health, Research, or related field
- Strong understanding of dementia care frameworks and aged care quality standards
- Experience in governance, advisory committees, research, or quality improvement initiatives
- Knowledge of contemporary aged care reforms and community care models
- Excellent communication, stakeholder engagement, and analytical skillsCommitment to person-centred, evidence-based care practices

Desirable

- Tertiary qualifications in Social Work, Aged Care, Disability Services, Community Services Coordination, Nursing or Allied Health discipline with a current AHPRA registration, or equivalent experience
- Experience in Support at Home, Home Care Packages, or community aged care
- Experience working in case management or care management in either Home Care Packages/Support at Home Program or similar
- Clinical background in geriatrics, nursing, psychology, or allied health
