The Village Baxter proudly supports older Australians through the provision of aged care and retirement living services from our Frankston South work premises.
We are currently seeking a dedicated and experienced Support at Home Care Partner to play a key role in the delivery of quality aged care services across Frankston and surrounding suburbs.
About the Role
As a Care Partner you will be responsible for the care management of Support at Home clients. This includes client assessment, care planning, ongoing review, and coordination of services to ensure the delivery of high-quality, person-centred support.
You will liaise with other Care Partners, Clinical Care Partners, coordinators, carers, and other healthcare professionals, and contribute to service improvement and quality assurance. This is a rewarding opportunity to make a genuine difference in the lives of older people.
This role is permanent, full-time and based on-site. Standard hours of work are Monday to Friday 8:24am to 4:30pm.
Key Responsibilities
Conduct comprehensive assessments and develop individualised care plans
Monitor and review client needs and adjust services as required
Ensure the health, safety, and wellbeing of clients and staff
Liaise effectively with clients, families, healthcare professionals and other stakeholders
Ensure timely completion of documentation and reports
Participate in quality assurance and continuous improvement initiatives
Promote a positive image of our services to the community
Once trained, to provide on-call support as part of a rotating phone roster
Undertake other duties as directed by the Community Services Manager
Who We’re Looking For
A skilled Care Partner with at least two years’ experience in a similar role, including a focus on client assessment and documentation
Someone with a relevant qualification in Aged Care, Community Services or equivalent (minimum Certificate IV) and a current First Aid Certificate
Excellent communication, interpersonal, organisational and digital technology skills with a commitment to maintaining high standards of safety, dignity, confidentiality, and person-centred care
Someone with a current full Australian driver's license with access to a reliable, registered and fully-insured vehicle
Understanding of OH&S; and infection control practices and an ability to respond capably and appropriately in emergency situations
Additional qualifications or training in gerontology will be highly regarded
